Monday-Friday 8:00 a.m. to 5:00 p.m.
Intake Process:
Must complete a program application and submit to main office in Ybor in person at 1205 E. 8th Ave Tampa, FL 33605 or mail to 1915 E 131st Avenue apt#116 Tampa, FL 33612. Applicant will be placed on waiting list. Applicant will complete a screening interview and will have a secondary group interview if being considered for the program.

Eligibility:
The veteran must have EITHER a substance abuse or mental health diagnosis and at least 30 days clean from substances. They must also be homeless. A complete referral packet, with health screening and certification of homelessness.
